| 内容記述 | 【要旨】2011年，大阪市あいりん地区にあるホームレスを支援する施設，シェルター，診療所，および，サンフランシスコ市の社会施設，病院，教会を視察した。その後，8年が経過し，結核やエイズに関する公衆衛生施策，栄養指導等の公衆衛生活動が行われた結果，両市で結核の患者数は減少していた。我が国では専門職が中心になってDOTSを行っているが，アメリカでは医療資格がない者が訓練を受けてDOTSを行っており，DOTSに関する国の経済的負担は少ない。しかし，サンフランシスコ市のような大都市では手頃な価格の住宅の欠如，精神疾患，暴力，エイズ，薬物等の問題がある。また，アメリカの企業は終身雇用ではないため，突然解雇されてホームレスになることがある。経済的な格差は健康格差を生むため，健康，労働，教育，福祉などさまざまな側面から公衆衛生活動について考える必要がある。蔑視せず，現実をしっかりと見て，彼らに寄り添い，今後どのようにしたらいいのかを共に考えなければならない。このような状況の中でアウトリーチ・ワーカーやボランティア等，専門家ではない一人の人として関わりや信頼関係の構築が，ホームレスにとって他のどのような支援よりも遙かに大きな力になっている。差別することなく共に考えることが一番の支援である。 | |||||||||||||||||

| 内容記述 | Abstract: This study discusses regarding the current situation of homeless and public health issue in urban area of Japan and the United States. The definition of homeless in Japan and the U.S. is basically different. Particularly, the United States has a different point of view for the people who lives in shelters and specific accommodations for homeless in Japan. In 2011, the authors inspected Mission Neighborhood Health Center (MNHC), a hospital which goes on Directly Observed Treatment Short-course (DOTS), Glide Memorial United Methodist Church, and personal house who takes outreach service and Castro District in San Francisco. Furthermore, the authors inspected a shelter for homeless in Osaka to confirm the life style and living situation of homeless. Eight years later, as results of the public health activities regarding tuberculosis and AIDS, nutritional counseling, the number of tubercular patients in both cities were getting decreased. DOTS is considered as an effective treatment in the health improvement of homeless in Japan. In Japan, DOTS are mainly operated by specialized personnel in Japan. On the other hands, in the United States, well trained staff who does not have medical license goes on DOTS. It reduces the national budget in the U.S. However, in metropolis like San Francisco, lack of reasonable residences, mental diseases、violence, AIDS and abuse of illegal drugs are always present as serious social issue. Furthermore, fires can happen to anybody because of the lack of idea for permanent employment in the U.S. Since the economic disparities induce the health disparities, adequate discussion regarding public health activities should be done from various viewpoints as health, labor, education and welfare. It is necessary for us to have continuous discussions how we should do for homeless in future without discrimination. It is difficult for homeless to get enough idea and power for settling their economic and health problems. In such a social situation, building reliable relationship as a person as outreach worker, volunteer, who is not a medical specialist could be great enhancer for homeless to get them, rather than any other supports for homeless. Continuous discussion without discrimination could be a best support for them. | |||||||||||||||||
